POCUS Case: Severe Mitral Regurgitation Detected at the Bedside Following Pacemaker Lead Endocarditis

Clinical Presentation

A 78-year-old gentleman presented to the Acute Medical Unit with progressively worsening shortness of breath over several days.

His recent medical history was significant for pacemaker lead infective endocarditis, for which the infected pacing system had been removed and he had completed treatment. Given his recent cardiac infection, there was concern that his symptoms could represent heart failure, recurrent infection, or another cardiac complication.

On examination, he was breathless at rest with signs of pulmonary congestion. Although blood tests and chest radiography suggested heart failure, the underlying cause remained unclear.

The POCUS Assessment

A focused bedside cardiac ultrasound (POCUS) was performed immediately.

The examination demonstrated:

  • dilated left atrium
  • Severe mitral regurgitation with a large regurgitant jet on colour Doppler
  • Preserved left ventricular systolic function
  • No significant pericardial effusion

The findings raised strong suspicion of acute severe mitral valve dysfunction, likely related to previous infective endocarditis with valvular destruction.

Management

The patient was referred urgently to the cardiology team following the bedside findings.

A comprehensive departmental echocardiogram confirmed severe mitral regurgitation, and the patient was transferred for specialist cardiology assessment and definitive management.

Early recognition of the underlying pathology allowed prompt specialist involvement and avoided delays that could have occurred if treatment had focused solely on managing heart failure symptoms.

Learning Points

Patients recovering from infective endocarditis remain at risk of significant structural valve complications.

Bedside cardiac ultrasound enabled rapid identification of the true cause of this patient’s deterioration, directing immediate referral for specialist assessment rather than treating presumed decompensated heart failure alone.

POCUS Changed Management

Initial presentation: Progressive breathlessness following recent pacemaker lead infective endocarditis

POCUS finding: Severe mitral regurgitation with left atrial dilatation

Management change: Immediate cardiology referral and urgent formal echocardiography

Outcome: Prompt diagnosis of significant valvular disease and expedited specialist management


Key Message

Focused cardiac ultrasound is far more than an assessment of ventricular function. In experienced hands, it can identify major valvular pathology, recognise complications of infective endocarditis, and accelerate life-saving specialist intervention at the bedside.
